LONG AND SHORT TERM GOALS DESIRED FUNCTIONAL OUTCOME
Long Term Goal: Carter will produce age appropriate sounds during structured activities to improve effective communication.

Short Term Goal: Carter will produce /k, g, f, v, s, z, “j”, “th”, “ch”/ in the initial position, medial position and final position of single words, phrases, sentences and in conversational speech with 80% accuracy. 

PRESENT LEVEL
Carter has mastered /k, g, f, v/ in all positions of single words, phrases, sentences and in conversational speech. He ended the 2016 school-year mastering /s,z/ in sentences. We are currently focusing on these sounds along with s blends in conversational speech.

I started seeing Carter in September of 2015. He has made tremendous progress over this past school-year and continues to make improvements. His mother and father are a great source of support and encouragement for Carter. He is an absolute joy to work with! Keep up the hard work, CarCar!
